New Client Information
Getting Started with Arbor Therapy
Starting therapy should feel straightforward. Whether you already have a referral or you’re just beginning to explore service, our team can help you through the intake process and prepare for the first evaluation.
Four Steps to Get Started
1. Complete Your Intake Forms
Start by completing the appropriate intake form with information about the client’s health, development, and current needs. Arbor Therapy provides pediatric and adult intake forms in both English and Spanish.
2. Make Sure We Have a Referral
Arbor Therapy requires a referral on file before beginning services. If you don’t already have one, that’s okay! Our team can contact your primary care provider to request it. You can also reach out to your provider directly, which can help speed up the process.
3. We’ll Verify Your Coverage
Once we have the necessary information, our team will work on any required insurance authorization.
4. Schedule Your Evaluation
After we receive the necessary paperwork, referral, and insurance authorization, we can schedule your evaluation. Most initial evaluations are approximately 60 minutes and can take place in one of our clinics or through teletherapy when appropriate. After the evaluation, we’ll discuss recommendations and next steps for ongoing therapy.
